Methods of applying for disclosure of personal information, etc.

When requesting disclosure or corrections of your personal information or notifications of purpose of use, etc. please consent to the following items before applying.

When a customer makes a request to a service desk of the Company to which the customer has registered or provided personal information

  1. The personal information requested must be personal information which the Company has the right to disclose, etc. (This is the same as "retained personal data" as defined under the Act on the Protection of Personal Information.)
  2. We ask that requests for disclosure, etc. follow the methods (verification of identity, documents to submit, etc.) established by each service desk. Please confirm the correct procedure with each service desk.
  3. If you do not know the contact details, please contact the Company's customer support desk.

When making a request via the Panasonic customer support desk for personal information inquiries

I. About making requests

  1. The personal information requested must be personal information which the Company has the right to disclose, etc. (This is the same as "retained personal data" as defined under the Act on the Protection of Personal Information.)
  2. Requests are limited to individuals identified by the personal information retained by the Company (hereinafter referred to as "the individual") or their legal representatives (appointed by the individual or guardians or other holders of custodial authority, etc.).
  3. Please fill in the appropriate form and mail it to the Company's customer support desk. If disclosure is requested, use the 開示請求書[PDF:263KB]; if notification of purpose of use is requested, use the 利用目的通知請求書[PDF:264KB]; if amendments, additions, or deletions are requested, use the 訂正等請求書[PDF:265KB]; if suspension of use, erasure, or termination of provision of personal information to third parties is requested, use the 利用停止等請求書[PDF:257KB]. Requests are accepted only in writing. (At the customer's wish, the Company will mail out various request forms. Please make an inquiry to request this.)
  4. The expense to the Company of mailing out each form is to be borne by the individual. When mailing any kind of document to the Company, please use a method that retains a record of delivery, such as registered mail.
  5. The handling fee for requests for disclosure or notification of purpose of use is 700 yen (including consumption tax). This is required for each request. When mailing documents or forms to the Company, please include stamps or a postal money order equivalent to the amount of the 700 yen handling fee.* If the handling fee is insufficient, we will contact you. If the handling fee is not paid within one week after contacting us, the request will be deemed invalid.
    *Postal money orders are issued at the post office. Please note that any fees incurred for currency exchange must be borne by the customer.
  6. Making a combined request for disclosure and notification of purpose of use will incur a one-time handling fee of 700 yen.
  7. Once a request for disclosure or notification of the purpose of use has been made, even if the Company does not possess the individual's personal information, or cannot disclose it for legal reasons, the handling fee will be charged and will not be returned.
  8. When making a request for amendments, etc., please give detailed instructions about which personal information held by the Company is to be amended and how to make the amendments. If detailed instructions are not received, amendments cannot be made.
  9. If a request is made with respect to multiple companies (i.e. the Company and its affiliated group companies), in order to respond to the request, the customer's relevant personal information will be provided to each company (for examination by the department holding it, etc.).

II. Confirming identity of the individual and his/her representative

  1. When making a request, please submit documents to identify you or your representative (hereinafter referred to as "identification documents"). When mailing each request form, please also enclose the following identification documents (all of which list name, address, and date of birth).
  2. In the identification documents, any personal information other than name, address, and date of birth, as well as the issuing number, issuing date, and issuing authority (name of prefecture or municipality, etc.) for public documents such as driver's licenses, is not required to verify identity. We ask that you black out this personal information or otherwise ensure it is not visible before submitting.

  3. A letter from the individual authorizing the representative to act in this capacity (the individual must submit his/her seal impression and the relevant Certificate of Seal Registration. A sample letter from an individual authorizing a representative to act in this capacity is attached for your reference. If the representative is also a legal guardian, etc. please submit a certified copy or extract of the Family Register that shows this relationship.)

III. Notifications of Disclosure or Purpose of Use

  1. We will conduct notification of disclosure or purpose of use insofar as we can confirm that the personal information belongs to the individual, using various request forms and identification documents.
  2. Notifications of disclosure or purpose of use will be sent by registered mail to the address of the individual whose identity has been confirmed by various request forms and identification documents. However, for some services such as email magazine distribution services and web services, identity verification is carried out with user IDs, passwords, etc. Thus, in some cases, the service department may send emails, etc. directly to the individual's email address as entered on the request form, indicating the procedures for notification of disclosure or purpose of use. In such cases, please follow the procedures described in the email, etc. to verify identity.
  3. If a request is made with respect to multiple companies (i.e. the Company and its affiliated group companies), and if personal information is held by multiple companies, each such company shall send notification of disclosure or purpose of use as per 2. above.
  4. Generally, the period from receipt of each request to sending notification of disclosure or purpose of use should be no more than 10 business days (about 2 weeks).

IV. Amendments etc.

  1. We will undertake amendments, etc. insofar as we can confirm that the personal information belongs to the individual, using various request forms and identification documents.
  2. For some services such as email magazine distribution services and web services, identity verification is carried out with user IDs, passwords, etc. Thus, in some cases, the service department may send emails, etc. directly to the individual's email address as entered on the request form, which will describe the procedures for making amendments, etc. In such cases, please follow the procedures described in the email, etc. to verify identity.
  3. Except in cases as per 2. above, the Company will notify of the results of amendment, etc. of personal information (including cases where amendments were unable to be made) by registered mail sent to the address of the individual whose identity has been confirmed by various request forms and identification documents.
  4. If a request is made with respect to multiple companies (i.e. the Company and its affiliated group companies), and if personal information is held by multiple companies, each company shall follow the procedures listed in 2. and 3. above.
  5. Generally, the period from receipt of each request to sending the results of any amendment, etc. should be no more than 10 business days (about 2 weeks).

V. Deletion, suspension of use, or termination of provision of personal information to third parties

  1. In cases of deletion of personal information, suspension of use, or termination of provision to third parties, users may be unable to continue receiving the services they currently use.
  2. In some cases (such as when the Company holds a claim against the individual), even when a request has been made for deletion, suspension of use, or termination of provision to third parties of personal information held by the Company, this request may not be approved for some of this personal information.

VI. Other

  1. Personal information listed on various request forms and identification documents will be used only as required to undertake notification of disclosure or purpose of use or amendments, etc.
  2. Any request forms and identification documents submitted will not be returned. All request forms will be managed appropriately by the Company. (The Company discards identification documents after verification of identity has been completed.)
  3. In order to confirm the content, etc. of various request forms or identification documents, the Company may contact the individual or his/her representative by telephone, etc.
  4. With regard to procedures for disclosure and notification of purpose of use or amendments, etc., some or all of these details may be changed without prior notice. When requesting notification of disclosure or purpose of use or amendment, etc., please check for the latest procedures.
